Additional Supporting Documents Please submit the following documents together with your proposal: MDM Vendor Creation Form (for vendor registration purposes); Financial statements for the last two completed financial years (preferably audited); Certificate of Incorporation or valid Business License; United Nations Global Marketplace (UNGM) Registration Number. If your company is not yet registered on UNGM, you are encouraged to create a supplier profile through the https://www.ungm.org.
