In case of award, the suppliers should provide:  Certificate of origin.  Certificate of analysis for each batch. The supplier should accept, sign and comply to the following attached documents: 1. UN Code of Conduct. 2. Self-Declaration Form. Orders resulting from this Invitation to Bid will fall under WHO’s General Terms and Conditions as per Annex 3 in the ITB document.
