Every potential bidder is requested to fill in the attached 104 form, for each lot you wish to participate in (each lot should be filled in a separate sheet labeled 104_lot#_"bidders name, only fill in the columns highlighted in yellow.   Updated master sheet includes two new columns for delivery time in weeks and quantity of available units in stuck, make sure to provide accurate estimates. This bid is for Vendors able to ensure delivery to UNICEF warehouses in Ashdod or Gaza entry points, UNICEF will not support any vendors in the import and clearance of items. All Potential Bidders are requested to fill in the Attached 104 form (columns highlighted in yellow) for their bids to be valid, each lot should be filled in a separate sheet, indicating the lot number and name of vendor. We strongly advise bidders to thoroughly review all provisions of this invitation to bid to fully understand UNICEF's requirements for this Bid.
