We kindly request you to submit your quotation for the supply and delivery of 1000 Individual First Aid Kits (IFAKs) based on DAP delivery term including customs clearance to Port au Prince, Haiti within 4-6 weeks from the date of the Purchase Order signature, as detailed in Annex 1 of this RFQ. This RFQ comprises the following documents:    Section 1 - This request letter Section 2 - RFQ instructions and data Annex 1 – Technical specification for goods Annex 2 - Quotation Submission forms Please note that the quotation must be submitted using Annex 2: Quotation submission forms by the method and by the date and time indicated in Section 2. It is your responsibility to ensure that your quotation is submitted on or before the deadline. Quotations received after the submission deadline, for whatever reason, will not be considered for evaluation. Kindly ensure that your quotation is signed in pdf format.
